CHECK IN INFORMATION
Online check-in is offered to prevent teams from having to come to the Midlothian Athletic Club(Tournament HQ) on Friday night.
Teams may check in online by sending all of the required documents at the same time. ALL INFORMATION MUST BE COMPLETE! DO NOT SEND THE INFORMATION IF IT IS NOT COMPLETE
Here is what we need:
(2) copies of the official team roster signed by the club registrar
* one copy is turned in and kept by the site coordinator, the other copy must be with the coach on the sideline and presented upon request along with player passes
- Recreational player passes for each player signed by the club registrar. Rec passes are the only passes accepted!
- A signed medical release form is required for every player. The document does not need to be notarized.
Once the team manager or coach has all documents ready for submission, please upload using your personal dropbox account and share with firstname.lastname@example.org.
OR you may attend check-in night and present your documents during check in at the Midlothian Athletic Club on Friday, May 31ST.
Team check in will take place on Friday, May 31st from 5:00 PM - 7:00 PM at the Midlothian Athletic Club 10800 Center View Dr, Richmond, VA 23235.
